We provide analytic and technology infrastructure tools to help support our PHM activities across our network, including new tools for doctors and nurses to interact with patients and each other, and a suite of analytic tools to track and monitor our performance.
Providing Patients with New Ways to Manage their Health
Virtual visits, which occur either in real-time using a secure video platform or by email, is a new innovative approach for physicians and patients to communicate. These visits, available for select conditions, replace in-person follow-up visits and are more convenient for patients and clinicians. To help patients navigate a complex heath care system, we provide resources to help patients understand their healthcare benefits and a secure on-line portal called Patient Gateway where patients can obtain their personal health information and communicate with their physicians and care teams.
We have also equipped our patients with tools to better care for themselves. For example, we are using tablets and remote devices that allow patients to track and monitor their symptoms, pain, and other health information. This information is shared directly with the patients’ physicians and care teams to inform treatment. In addition, we are giving patients educational information to better inform and engage them in their treatment decisions.
Providing Clinicians with Next Generation Clinical Tools
Under accountable care payment models, providing clinicians with the right tools to manage their patients is critical. At Partners, we use registries for specific conditions to track patient outcomes. This allows us to actively monitor patients and identify and correct gaps in care quickly. To help understand which patients might need specialist input, but not require an in-person visit with a specialist, we developed referral management tools. Lastly, we have developed clinical decision support tools to assure that we are providing the most appropriate and evidence-based care. Our treatments are reflective of the latest clinical guidelines and patients receive the treatment that is right for them.
Using Analytics to Track Performance over Time
Advanced data warehouses and analytics are required for the success of population health management. To improve reporting and enable patient-centered coordination of care, we are transitioning to a single uniform platform (Partners eCare) that integrates health, administration and financial data into one system, ensuring that all providers have the most up-to-date information about their patients. To improve system-wide analytics and reporting, we have developed an enterprise data warehouse (EDW) that integrates data from multiple sources making it more readily accessible for analytic work. The EDW also offers a range of self-service analytical applications.
