Patient-Centered Medical Home: An Advanced Model of Primary Care
In 2012, Partners HealthCare embarked on a journey to transform our primary care practices to the Patient-Centered Medical Home (PCMH) model of care. We took a phased approach, asking practices to lay the foundational tools, structure, and capacity necessary to adopt the higher functioning components. Our four key aims for patient-centered care are:
- Enhanced patient experience of care
- Improved health of the population we serve
- Reduced health care costs
- Improved work life of health care providers and staff
Each year, we set goals directed towards these four aims for all of our primary care practices. As our practices meet these goals, they have achieved a term we internally call “Primed Status” – a step closer towards PCMH certification recognition by the National Committee for Quality Assurance (NCQA).
By the end of 2018, all Partners primary care practices will receive PCMH recognition. We have made substantial progress towards this goal of transforming our primary care practices.
