As an Accountable Care Organization (ACO) Partners-affiliated doctors, hospitals, and other health care providers, come together to provide coordinated, high quality care to our patients. Coordinated care helps ensure that our patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors.
Enhancing the Care You Receive
By participating in an ACO, your primary care physician has the opportunity to enhance the care you receive by better coordinating your care with your other health care providers. The goal of the Partners ACO is to deliver high quality care and better meet your individual needs and preferences. This is done by enabling your doctors to communicate closely with your other health care providers through enhanced sharing of your health care information.
What do you mean by “Accountable”?
Accountable Care Organizations have agreements with insurance companies to provide special services and care coordination for their patients. These agreements set performance standards in quality and spending for the ACO. If the ACO is able to meet or exceed these standards for patient care, they may be eligible for shared savings with the insurance company. If they do not meet the standards, they may have to pay money back to the insurance company.

By doing so we will be able to maintain and aim to improve the quality of your care.
Since 2012, Partners has entered into new contracts with all the major insurers including the Centers for Medicare & Medicaid Services (CMS), Commercial payers, and MassHealth. As of 2017, Partners has two ACOs with government payers: Partners participates in the Next Generation ACO model for Medicare patients and Partners Care Connect for MassHealth patients.
After four years in the Medicare Pioneer Accountable Care Organization (ACO) Model, Partners saved $31.5 million. With an overall quality score of 96%, Partners is among the best ACOs in the nation for delivering high-quality care to patients. In 2017, Partners became a Next Generation Medicare ACO, a new model geared toward health care organizations with previous experience managing large populations.
Our Medicaid ACO, Partners Care Connect, is part of the MassHealth pilot program; of the first Medicaid ACO pilots in the country.
